JMS Student Registration

Hello Jefferson Middle School Families,

 

We hope this email finds you safe and healthy. The online registration window is now open for the 2020-2021 school year. We are asking that all families upload appropriate documents instead of bringing them into the school. When online registration is complete and all documents have been uploaded, your student’s registration will be approved.

 

If your student is a current APS student, please log into your ParentVue and complete your online registration. If your student is brand new to APS, or returning from a charter or private school, you will first need to create a ParentVue account and start the process here:

If your student has been with APS previously, you can use their past student identification number. If you are unsure of their number, you can call or email the APS Service Center at (505) 855-9040 or servicecenter@aps.edu and they can help you retrieve it. If your student will be brand new to APS, then you will follow the instructions to create a new Activation Key.

 

FOR NEW AND RETURNING STUDENTS: 

Once you log into ParentVUE, you'll be asked enter student information including: 

• Address 

• Parent/Guardian(s) Name and Contact Information 

• Emergency Contacts 

• Health Information including the name of your healthcare provider, dentist, health conditions and/or medication (if applicable) 

• Immunizations 

• Information Release 

• Language Usage Survey 

• Previous Schools Attended 

• Parent/Guardian Relationships

 

Documents you will need to upload (through a scanner or photo): 

• Two Proofs Of Residency/Address (includes a current rental/lease or mortgage statement/agreement and a current utility bill). 

• Immunization Card 

• Custodial documentation (if applicable)

 

SOME IMPORTANT THINGS TO BE AWARE OF:

• It is required that you upload at least TWO documents to verify your address. Accepted documents are: current lease/rental/mortgage agreement, current utility bill, notarized family living with family form, driver’s license (paired with current lease/rental/mortgage agreement or current utility bill). 

• If you submitted your online registration before today and DID NOT upload documents, your registration will be re-opened and sent back to you so that you may upload all necessary documents. 

 

If there is anything missing or incomplete in your registration, it will be reopened and you will see a note as to what you need to do to complete it, or you may be asked to schedule a time to come in and complete your registration with a secretary.

 

If you are unable to upload documents, please email your student’s grade level secretary to receive technical support after July 13th.
