Health and wellness is the focus of Jefferson MS Health Office. Parents have the basic responsibility for the Health of their children. Parents are encouraged to:
- Share their student's health history
- Keep student's health records and emergency contacts up-to-date
- Provide required documents - New Mexico
- Asthma Action Plan, Allergy Action Plan immunizations
- Keep students home when ill.
- Encourage healthy sleep, nutrition and exercise.
The Health Office is NOT a replacement for your usual provider (MD, NP, PA) or emergency services. Excused absences are from providers only, not the Health Office. If you have a medical emergency, please call your provider or 911.
For emotional or mental health issues, contact New Mexico Crisis and Access Line 1-855-NMCrisis (662-7474).
Suicide Prevention: Dial 988.
The Health Office provides:
- First aid and safety
- Assessment of illness or injury
- Immunization records
- School health records
- Medication administration
- Vision and hearing screening
General Health Office Guidelines
New Mexico law required all students be currently immunized or in the process before entry into school. Exemptions may be granted for religious or medical reasons. Form to complete:

Please inform the school nurse of any health concerns, medical diagnoses, medication the student takes, allergies. Provide the Health Office with the necessary Asthma Action Plans and Allergy Action Plans.
Prescription and over-the-counter medications can be stored in the Health Office and given as prescribed. Forms for prescription medications and OTC medications must be signed by the parent/guardian.
School-Based Health Care
Jefferson Middle School students can use the health clinic at Washington Middle School. The clinic provides behavioral health services for emotional wellness and medical care for health care services including wellness exams, vaccines, sports physicals, injury and illness care, and other services. These services are provided regardless of your ability to pay. Call for an appointment at 505-248-1116.
