All Sports Requirements

• All sports require students to receive a physical and be cleared to play. (Turn-in the physical to Mr. Wade)

All of the forms you will need can be found on the school website in the athletics section: All must be complete and turned in to the Athletic Director (TBA) to be eligible.

REQUIRED: Complete the Concussion for Students course on nfhslearn.com. You must create an account and click on Order Course (no charge). Print the certificate when the course is completed or e-mail it to the Athletic Director - Bo Wade - bo.wade@aps.edu.
